People with locked-in syndrome are conscious and can think and reason, but not in a position to speak or move.
You can communicate with that person by flashing to. You can say to the patient. Can you hear me? Blink twice, yes. If the patient responds, then you can even without flash, and thereby keeping them on a limited conversation.
